ACT.md: A Community Care Coordination Platform


NCHIIN is pleased to support community efforts to coordinate care for patients with complex needs.  Stakeholders in Humboldt County have long recognized the need to develop infrastructure to support data sharing and care coordination across the multiple entities involved in a patient's care.  NCHIIN is working to address this need by implementing a community care-coordination platform in Humboldt County called ACT.md.
ACT.md is a browser-based and mobile-friendly HIPAA compliant platform.  ACT.md has interoperability with NCHIIN’s HIE to allow for data exchange across community organizations.

The ACT.md platform allows for:

  • Standardized workflows
  • Platform-based assessments and forms
  • Secure Messaging
  • File management

 



The platform is currently being used by Humboldt County Department of Health and Human Services, St. Joseph Health-Humboldt County’s Care Transition and Paso a Paso programs, and the Redwood Community Action Agency.  NCHIIN aspires to work with community partners to bring the platform to scale across the community.
