The CIE (North Coast Care Connect) supports cross-sector care coordination by enabling participating organizations to:

  • Send and receive referrals
  • Document interactions and services
  • Share care plans and track outcomes
  • Coordinate across health, social service, and community partners

  • Referral workflows and coordination questions
  • Data entry, forms, or reporting issues
  • Training or user support needs
