NCCC Tip of the Week: Simplify Collaboration by Building a Strong Care Team!
Creating a Care Team for your clients is a great way to ensure they receive the best support possible. By inviting key team members, you can improve coordination, enhance communication, and provide a more holistic approach to care.
Here’s how to quickly add users to a Care Team:
Steps to Invite Users to a Care Team:
➡️ Go to the Client Profile.
➡️ Click the “Add Care Team Member” button under the Care Team list.
➡️ Choose whether the new team member is from your agency or an external agency:
- If from your agency, you can:
✅ Add them immediately by clicking “Add to Care Team” or
✉️ Send them an invite, allowing them to accept or decline
- If from an external agency, send an invite to collaborate.
➡️ Invitations will appear in your To-Do List as Pending until the user takes action. You’ll be notified once they accept, assign, or decline the invitation.
📢 Important: If the client has chosen client-level consent, they will receive a text message or email asking them to approve or decline the new Care Team member.
By building a strong Care Team, you help streamline referrals, avoid service gaps, and ensure that everyone involved in a client’s care is on the same page. Try it out and see how seamless collaboration can be!
